Risk Of Fatal Heart Attack Underscores Importance Of High-Dose Methadone Detox

Patients taking higher doses of methadone — 120 mg per day or more — are at increased risk of sudden death from heart attacks, according to new research. This news only underscores the vital importance of high-dose methadone detox programs for anyone dependent on high doses of methadone.

A research team, at Beth Israel Medical Center in New York City, grouped chronic pain patients according to methadone dose. The group on daily doses above 120 mg showed significantly higher risk of “prolonged QT interval”, an electrical abnormality in the heart rhythm. This abnormality can lead to a sudden onset of ventricular tachycardia, a type of rapid heartbeat which is often fatal.

“There is a dose-dependent effect in patients taking more than 120 mg per day,” said Ricardo Cruciani, MD, the leader of the research team, in a report to the American Pain Society 30th Annual Scientific Meeting.

Methadone tops the list of opioid deaths

According to the Centers for Disease Control and Prevention (CDC), fatal opioid overdoses have tripled in recent years, and methadone deaths top the list. One of the reasons for this, Dr. Cruciani said, is the increased use of methadone to treat chronic pain, because of its lower cost and longer duration of action.

A research team from the National Center for Health Statistics at the CDC pointed out that methadone is a long-acting opioid that requires a complex dosing schedule.

“Methadone relieves pain for 4 to 8 hours, but remains in the body for up to 59 hours,” Dr. Diane Makuc, the CDC team leader, said. “A lack of knowledge about the unique properties of methadone was identified as contributing to some deaths.”

A study last year of opioid overdose deaths also concluded that a disproportionate number of deaths are related to methadone. Investigators reported that 30 percent of deaths involved methadone, even though methadone represents only 5 percent of the nation’s opioid prescriptions.
